Founder & Clinical Counselor
Stephanie Yates, MA, LCPC
Stephanie (she/her) holds a Master of Arts in Clinical Mental Health Counseling from Northwestern University in Evanston, IL. She has been working in various counseling settings for 10 years. Stephanie sees individuals, couples, and families.

Clinical Director & Counselor
Jay Swanson, MEd, LCPC
Jay (he/him) holds a Master of Education in Clinical Mental Health Counseling from DePaul University in Chicago, Illinois. He has been a Licensed Clinical Professional Counselor (LCPC) in Illinois for 5 years, primarily working in outpatient counseling settings. Jay started working in substance use counseling, where he regularly worked with clients with dual diagnoses, and then shifted to clinical counseling, where he has worked with clients ranging from children to older adults. Jay is experienced in facilitating group and individual counseling.
Jay strives to foster an inclusive, affirming, and compassionate environment. He believes that the path to healing involves clients learning about themselves, how their past impacts their present functioning, and learning skills to grow and achieve their goals. He believes that therapy is not “one size fits all” and utilizes a variety of therapeutic modalities depending on what clients need. He is passionate about working with members of the LGBTQ+ community in particular, as he believes that working with someone who is a part of the community and has lived experience is invaluable. Jay works with adolescents and adults of any identity in an individual setting.
In his spare time, Jay enjoys making collages, cross stitching, and listening to audiobooks.

Counselor
Andrew Culhane, MA
Andrew (he/him) holds a Master of Arts in Clinical Mental Health Counseling from Northeastern Illinois University in Chicago, IL. Andrew works with teens and adults in individual counseling.


Clinical Counselor
Tristin Capelle, MS, LCPC
Tristin (she/her) is dual-licensed in Illinois and Oregon as a Licensed Clinical Professional Counselor (LCPC) and Licensed Professional Counselor (LPC). She brings more than seven years of combined experience in mental health treatment, substance use treatment, case management, and higher education settings. Tristin earned a Bachelor of Arts in Women and Gender Studies from Arizona State University and a Master of Science in Professional Counseling from Grand Canyon University.

Tristin is skilled in developing evidence-based treatment plans, conducting diagnostic assessments, and using therapeutic approaches that support client engagement, primarily CBT and DBT. She values cultural competence, ethical practice, and confidentiality.
Outside of her clinical work, Tristin enjoys being an auntie, taking her dog to the park, and resale or bargain shopping.

Clinical Intern
Ant Walach
Ant (he/they) is a Master of Arts candidate in Clinical Mental Health Counseling at Northwestern University in Evanston, IL. He works with children and adults navigating identity, life transitions, and a range of personal concerns, with particular experience supporting members of the LGBTQ+ community. After completing his internship and graduating this fall, he looks forward to continuing his clinical work as a counselor at Mosaic.
Ant's therapeutic approach is collaborative, drawing on psychodynamic, person-centered, and Acceptance and Commitment Therapy (ACT) frameworks. He tailors his approach to each client, helping them build awareness of their patterns and clarify their values, and is committed to creating an affirming, inclusive space where clients feel safe bringing their whole selves to the work.
Before counseling, Ant spent two decades in the specialty coffee industry as a roaster and educator. He and his wife co-own Snowdrift Coffee Roasters, which he plans to continue running alongside his counseling work.
In his spare time, Ant can be found working on his 1996 Ford Ranger or 1950 Chevy, tending to houseplants, or spending time with his wife, their cats (Gary and Eric), and their chiweenie, Penny.

Clinical Intern
Jade King
Jade (she/her) is a Master of Arts candidate in Clinical Mental Health Counseling at Northwestern University in Evanston, IL. Jade believes that counseling spotlights the strengths and power that clients already possess and that it helps clients navigate life’s stressors through clinically trained support. When she graduates, she plans to work with survivors of domestic violence and sexual assault in the Rockford area to help them claim back their power and autonomy. She hopes to work in both shelters and private practice. She wants to help survivors discover their inner resilience, cope with their trauma, and regain control of their lives. She has worked in domestic violence shelters for over three years and is passionate about this population.
Jade will be able to work with adult clients individually when she starts her internship.
In her free time, Jade likes to crochet, paint, play video games, and play with her beautiful cat Magnolia.


Office Manager
Connie Yates
Connie (she/her) works as Mosaic's office manager. She is usually the friendly voice on the other side of the phone. She handles client calls, concerns, inquiries, and schedules appointments.
